GDPR, OWNER’S RELEASE & CONSENT
I consent that my details, and any data relevant to my dog using Love Pooches services, will be held on record at the address of the business’ property and be processed under the terms of the General Data Protection Act 2018.
Under GDPR the business agrees to keep all my personal details confidential & dispose of them correctly after three years where necessary.
However, I understand that this data may be shared with colleagues, or relevant agencies, i.e. Veterinary staff, Police, RSPCA or rescue centres, Council & Government Departments if requested.
By signing this dog boarding form, I am agreeing to, and understand that I am contractually bound
by the business’ terms and conditions. I fully agree that the information recorded on this document during my dog’s meet and greet with the business is accurate.
I, hereby confirm that I am the owner of the named pet written at the beginning of this dog boarding form, and I authorise Jocelyn Baker @ Love Pooches to act as guardian during my absence and agree for her/him to make any decisions regarding my dog’s health providing it is acting in the best interest of my dog, and is on the advice of the registered veterinarian with whom Jocelyn Baker @
Love Pooches is registered with…and by circling YES or NO, to the specific activities under the owners consents & by signing this DOG REGISTRATION BOARDING document.
I agree that the information given on this form is accurate. I agree that if my dog harms another dog, or animal, or a human whilst in Love Pooches care that I will be held liable for any costs, especially if I have not declared any historical negative behaviours such as attacking or biting. I therefore will not, nor cannot hold Jocelyn Baker @ Love Pooches responsible for any :
• accidental injury to my dog,
• injury to another animal by my dog,
• injury to a member of Love Pooches
• unforeseen issues, or
• death of my dog
nor will I make a claim on the business’ insurance company if any issues occur, unless Jocelyn Baker
of Love Pooches is found negligent.
